If you would like Schlotzsky's to become a sponsor of your program, event, or organization, please complete this application and mail or fax the request form, along with a cover letter on your organization's official letterhead. Please include a copy of your 5.01(c)3 certification, if applicable. This request form should be submitted at least four weeks prior to your event. The form includes further instructions. The submission of this request form does not obligate Schlotzsky's, Ltd. in any way or manner.
